    5th European Symposium on Transition

    25-26 september 2025

    The European Symposium on Transition is a biennial conference where researchers and professionals can share their knowledge, learn, and empower each other to further improve transitional care for young people with chronic conditions or disabilities.

    Transition refers to the purposeful and planned process of moving from paediatric to adult care services beginning in adolescence and continuing into early adulthood. Transition needs to be an individualized, coordinated and collaborative process between the youth, family and paediatric and adult care providers. The 5th edition will be held in Paris, France on 25-26 September 2025 and will focus on the current challenges and outstanding questions in transition care.

    Previously, the Symposium has been organized in Lausanne, Switzerland (Joan-Carles Suris, Christina Akre) and Rotterdam, Holland (Anneloes Van StaaJane Sattoe).

    The symposium is ideal for researchers and healthcare professionals (physicians, nurses, psychologists, physiotherapists, social workers, etc.) who work in somatic healthcare, rehabilitation and mental healthcare with adolescents and young adults with chronic conditions or disabilities, and
    wish to develop their knowledge and share their experience in the field. It is also of interest to healthcare decision-makers and patient groups advocating for adolescents and young adults with chronic conditions or disabilities.
